چکیده

In 2010 the SAFE-TRIPS investigators reported on resuscitation fluid administration in 391 intensive care units (ICUs) in 25 countries.(1) The study found that more than one third of patients in the ICUs received resuscitation fluids on the study day and that the choice of fluid administered was determined by local practice and habit rather than by any identifiable patient characteristic. Given that many thousands of patients receive resuscitation fluid each day, it is an intervention that has the potential to result in either great benefit or great harm. Although recently published trials by clinician investigators have dramatically increased the evidence base in this area,(2-9) some questions remain unanswered.

Different effects of fluid loading with saline, gelatine, hydroxyethyl starch or albumin solutions on acid-base status in the critically ill

INTRODUCTION Fluid administration in critically ill patients may affect acid-base balance. However, the effect of the fluid type used for resuscitation on acid-base balance remains controversial. METHODS We studied the effect of fluid resuscitation of normal saline and the colloids gelatine 4%, hydroxyethyl starch (HES) 6%, and albumin 5% on acid-base balance in 115 clinically hypovolemic cri...

Metformin Treatment in Hyperglycemic Critically Ill Patients: Another Challenge on the Control of Adverse Outcomes

New-onset hyperglycemia in patients admitted to intensive care units increases the risk of morbidity and mortality. Insulin resistance is frequently seen in the treatment of stress-induced hyperglycemia. Metformin, an oral anti-hyperglycemic agent, may introduce a new treatment protocol in critically ill patients with insulin-resistance hyperglycemia. Fifty-one non-diabetic traumatized patients...

Fluid therapy in equine patients: small-volume fluid resuscitation.

Fluid therapy is an important component of resuscitation and treatment of critically ill equine patients. However, the decision of which fluid type to use in certain critical situations is somewhat controversial. This article focuses on the potential benefits of performing small-volume resuscitation in critically ill horses by combining hypertonic saline with a colloid (e.g., hetastarch).

Fluid balance as a biomarker: impact of fluid overload on outcome in critically ill patients with acute kidney injury

Fluid therapy is fundamental to the acute resuscitation of critically ill patients. In general, however, early and appropriate goal-directed fluid therapy contributes to a degree of fluid overload in most if not all patients. Recent data imply that a threshold may exist beyond which, after acute resuscitation, additional fluid therapy may cause harm. In patients with acute kidney injury and/or ...
